The indicative list of documents required for each type of loss is mentioned below:


Medical Expenses

  • Future Generali Travel Suraksha Claim Form
  • Policy Copy / Certificate Copy
  • Treating / attending Doctor’s report
  • Original Admission / discharge card
  • Original Bills with Receipts & Prescription
  • Original X-ray reports/Pathological / Investigative reports, if any
  • Copy of passport / Visa with Entry & exit stamp
  • Copy of ticket and boarding pass

Accidental Dismemberment / Disablement

  • Future Generali Personal Accident Claim Form
  • Policy / Certificate Copy
  • Attending Doctors Report
  • Original Disability Certificate from the Doctor, if any
  • Investigation / Lab Reports (X-Rays & reports etc)
  • Original Admission / Discharge Card, if hospitalised
  • Police Inquest report, where applicable
  • In case of PTD claims – Disability Certificate from treating doctor.

Accidental Death

  • Future Generali Personal Accident Claim Form
  • Policy / Certificate Copy
  • Attending Doctors Report
  • Original Death Certificate
  • Original / attested Post Mortem / Coroners Report
  • Attested copy of FIR / Panchnama
  • Police Inquest report, where applicable

Repatriation of Mortal remains

  • Future Generali Travel Suraksha Claim Form
  • Policy / Certificate Copy
  • Hospital admission / discharge card, if hospitalised
  • Medical Reports / Investigative (coroners / Post mortem) Reports
  • Death Certificate
  • Funeral Certificate along with original bills / receipt towards funeral expenses.
  • Copy of passport / Visa

Medical Evacuation / Transportation to Republic of India

  • Future Generali Travel Suraksha Claim Form
  • Policy / Certificate Copy
  • Hospital admission / discharge card, if hospitalised
  • Medical Reports / Investigative (coroners / Post mortem) Reports
  • Doctor’s statement indicating the cause of illness and necessity of medical evacuation / transportation to the Republic of India.
  • All original bills / receipt towards expenses incurred during medical evacuation / transportation.

Loss of Checked in Baggage

Delay of Checked in Baggage

  • Future Generali Travel Suraksha Claim Form
  • Policy / Certificate Copy
  • Property Irregularity Report (obtained from Airline)
  • Copies of Correspondence with the Airline authorities / others confirming the loss and details of compensation.
  • Original bills / receipts / invoices pertaining to expenses incurred / purchases made towards necessary personal effects, during the delay period.
  • Copy of the passport / Visa with Entry & exit stamp.

Loss of Passport

  • Future Generali Travel Suraksha Claim Form
  • Policy / Certificate Copy
  • Copy of new passport & previous passport (if available)
  • Original bills / invoices of expenses incurred for obtaining a new passport
  • Copy of FIR / Police Report

Compassionate Visit

  • Future Generali Travel Suraksha Claim Form
  • Policy / Certificate Copy
  • Certificate from treating doctor stating the reason for such assistance.
  • Copy of Air Tickets
  • Original Bills / Invoices towards expenses incurred.

The list given is indicative in nature. Further additional documents may be called for depending on the nature of the claim.
